Job Summary: The resident service designee will assist the Director of Resident Service by acting as a liaison and representative of the resident’s social interest in accordance with current federal, state and local standards governing the facility, and as may be directed by the Administrator, to ensure that the medically-related emotional and social needs of the residents are met and maintained on an individual basis.
Job Functions: To gather clinically relevant information and completes psychosocial history to assist the Resident Service team in developing treatment plan and determining services needed by residents/families. Actively collaborates with the Resident Service team to meet resident care, outcome management and system improvement goals.
